If you're stuck for what to write, most of my clients do the following:

• In the first paragraph, tell me what is the greatest challenge you face today.
• Next, use complete sentences to describe for me what you consider to be your three best qualities or personal traits.
• Next, tell me what you would choose to accomplish if you could do anything you wanted.
• And finally, sign your name and the date.
